We provide online telehealth services that allow our members to submit their medical history and connect with independent medical professionals ("Medical Professionals") for medical and wellness services. We also provide mental health consultations from counselors ("Counselors") who do not provide professional or clinical services.
Medical professionals delivering services through Patient to Prescription are independent practitioners operating within affiliated independent professional medical groups (collectively, the "On-Demand Providers"). Please refer to the applicable Notice of Privacy Practices for On-Demand Providers for additional information about these groups and their organization.
Please note that users in different regions may be subject to different data protection laws, regulations, and standards. Accordingly, this document includes a section applicable to California residents and a section applicable to individuals located within the European Union.

By accessing or using the website or the Services, you agree to the practices described in this Privacy Policy, as well as the Patient to Prescription Terms of Service and any other applicable terms, agreements, notices of rights, consent forms, and policies, as applicable.
Geographic Scope: The Patient to Prescription Services are intended for individuals located in the United States, and we primarily process personal information in accordance with applicable United States privacy and healthcare laws. If we are subject to non-U.S. data protection laws with respect to specific users, services, or processing activities, we will comply with those laws as required. State-Level Applicability: This Policy is designed to apply to operations in all 50 U.S. states and incorporates enhanced protections required under applicable state privacy laws.

We collect the following categories of information from members, users, employers, and program partners:
- Identifiers: name, address, email address, phone number, date of birth, Social Security number (last four digits only, where required for identity verification or plan administration and permitted by law).
- Demographic and Eligibility Information: employer or plan details, coverage information, eligibility status, and member identification numbers.
- Health Information: medical history, symptoms, diagnoses, prescriptions, clinical records, insurance claims data, and communications with medical professionals.
- Technical and Usage Data: IP address, device type, browser type, mobile device identifier, cookies, web beacons, and other analytics information.
- Communications and Interaction Data: messages, customer support inquiries, or chats conducted through our applications or user account portal.
We collect information directly from you, from your health plan or employer (where permitted by law or contract), from your medical providers, and through technologies that support our platform.

We use personal data and health information for legitimate business purposes, including:
- Providing and coordinating clinical and telehealth services;
- Managing member accounts and providing customer support;
- Processing payments and insurance claims;

We do not use or disclose protected health information for marketing or commercial purposes without your authorization.
