Terms of Service

Patient to Prescription does not practice medicine or any other licensed profession and does not interfere with the practice of medicine or any other licensed profession by healthcare professionals, each of whom is solely responsible for the services they provide and for complying with the requirements applicable to their profession and license. Neither Patient to Prescription nor any third parties that advertise the Services or provide you with a link to the Services are responsible for any professional advice you receive from a healthcare professional through the Services.

Personal information that we receive or obtain as a result of your use of the Services may be subject to various state and federal privacy laws, including the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations ("HIPAA"). The descriptions in the Terms and our Privacy Policy regarding our collection, use, and disclosure of personal information may be governed or further limited by the requirements of such laws. In addition, in circumstances where Patient to Prescription collects, creates, receives, maintains, uses, or discloses protected health information ("PHI"), which is individually identifiable health information protected under HIPAA, we may be and likely are subject to certain agreements, including agreements with health plans and healthcare providers. In such cases, our collection, creation, receipt, maintenance, use, and disclosure of PHI will be carried out in accordance with those agreements and the applicable Notice of Privacy Practices of your health plan, where and when applicable. For official notices, as well as for additional information or answers to questions regarding the use and disclosure of your medical information, please refer to the Notice of Privacy Practices of your health insurance plan, if applicable.

User Accounts

Registration with Patient to Prescription is not required to access our website. However, in order to access the Services, you must register on the website. When registering on the website, you will be required to create an account ("Account") by providing your name, email address, password, and certain other information collected by Patient to Prescription (collectively, "Account Information").

To use the Services, you must create an Account and agree to these Terms of Service. You agree that the Account Information you provide to us at any time, including during registration and any information you upload to the website, will be truthful, accurate, current, and complete. You may not transfer or share your Account password with anyone or create more than one Account (except for sub-accounts created for children for whom you are the parent or legal guardian).

You are responsible for maintaining the confidentiality of your Account password and for all activities that occur under your Account. You agree to notify us immediately of any unauthorized use of your username, password, or any other breach of security of which you become aware in connection with the website by contacting us using the contact information provided below. Under no circumstances shall we be liable to you for any obligations or damages arising out of your use of the website, your use of Account Information, or your disclosure of Account Information to any third party. You may not use another person's Account.

Applicable law may not allow the limitation or exclusion of liability for incidental or consequential damages, so the above limitation may not apply to you.
